About the Opportunity The Community Participation Coordinator is responsible for developing, coordinating, and evaluating a diverse range of social, recreational, and community-based activities for clients funded through the Commonwealth Home Support Programme (CHSP). The role combines program coordination, case management, stakeholder engagement, and client support to ensure programs are person-centred, responsive to client needs, and delivered safely and effectively. - Develop and coordinate a varied calendar of social, recreational, and community participation activities
- Promote social connection, wellbeing, and independence for older people
- Coordinate day-to-day activity scheduling, logistics, and service delivery
- Build strong relationships with clients, families, volunteers, community groups, and service providers
- Conduct client assessments and reassessments relating to social participation needs
- Maintain accurate care documentation and activity plans
- Monitor program attendance, quality, and client outcomes
- Manage activity budgets and monitor expenditure
- Support continuous improvement and compliance with aged care standards and CHSP requirements
About You To be successful in this role, you will have: - Demonstrated experience in case management, community services, aged care, or a related field
- Experience undertaking assessments, service planning, and reviews for older people
- Strong organisational, communication, and relationship-building skills
- Experience coordinating programs, activities, or community-based services
- Sound understanding of aged care legislation, funding, standards, and best practice
- Ability to manage competing priorities and work autonomously
- A commitment to person-centred care and supporting older people to achieve their goals
To be considered for this role, you will have: - Diploma or Degree in Human Services, Health, Community Services, or a related discipline
- Experience working within the (CHSP) Commonwealth Home Support Programme (desirable)
- Minimum 2 years' experience in case management and service coordination for older people (desirable)
- Current Driver's Licence
- Registered and roadworthy vehicle
You must also be willing to obtain a National Police Certificate before commencing employment as well as a functional assessment.
