About the Role The Support at Home Care Partner plays a crucial role in ensuring the well-being and support of older people. You will develop and manage individualised care plans and budgets in collaboration with clients, their families, and healthcare providers. You will support the delivery of high-quality home care services and provide guidance to home care team members and clients. Responsibilities include but are not limited to: - Develop, maintain, and update individualised care plans with clients, families, and healthcare teams
- Monitor service delivery and reassess care needs as required
- Provide advice on service selection, self-management, and future care options
- Act as the primary contact for clients and families, addressing concerns and facilitating access to services
- Manage budgets, monitor funding, and meet productivity targets
- Maintain accurate written and electronic records in line with legal and professional standards
- Deliver services according to organisational policies, legislation, and Aged Care Standards
- Support continuous improvement and ensure property and equipment are secure
- Build and maintain positive relationships with consumers, families, service providers, and government agencies
- Promote a consumer-focused culture and resolve issues efficiently with empathy and transparency
About YouEssential Requirements: - 2+ years’ experience in case management, assessment, planning, and evaluation of services for older people
- Knowledge of aged care legislation, funding, policy protocols, and duty of care obligations
- Current driver’s licence and registered, roadworthy vehicle
Desirable: - Diploma or degree in Human Services, Health, Community Services, or related field
- Highly developed interpersonal, communication, and negotiation skills
- Basic knowledge of legislation such as Elder Abuse, Code of Conduct for Unregistered Health Practitioners, and duty of care.
- Demonstrated experience in all aspects of case management including comprehensive assessment, re assessments, planning of services within budget, problem solving and the provision and evaluation of services for older people
You must also be willing to obtain a National Police Certificate before commencing employment as well as a functional assessment.
